2. Go back to the OnPatient email you received, and click the pink SIGN UP NOW button, to sign up for our patient portal and enter your personal information:
3. Next, please visit our website www.Compbreastcare.com. On the homepage you will see a box titled Risk Assessment, and you will click the Learn More button to complete your personalized Cancer Risk Assessment.
- This assessment tool allows our team to understand your underlying breast cancer risk and family history. The risk assessment questionnaire can be completed in less than 10 minutes. When the questionnaire is completed
you do not need a password, though you will be asked for one.
